How to Get Credentialed with Blue Cross Blue Shield of Arizona (AZ Blue)
By Cactus Billing Solutions, Tempe, AZ · Updated October 2026
Blue Cross Blue Shield of Arizona, which now calls itself AZ Blue, is a major commercial insurer in Arizona, so getting into its network matters for most practices here. The process is well documented but strict: AZ Blue will reject a request that is missing information. Here is how it works, based on AZ Blue’s own Network Participation page and its 2026 Provider Operating Guide.
First, check whether you are already in network
Search the AZ Blue Provider Directory before you apply. If you are already credentialed and only need to add a tax ID or update details, AZ Blue wants a Provider Information Change Form, not a new application. To add a line of business or join another AZ Blue network, contact your assigned Provider Relations Contact.
Pick the right request form
AZ Blue has separate forms depending on who is applying:
- Medical practitioners: initial credentialing for providers not yet participating. This form requires a CAQH Provider Data Portal profile and CAQH ID.
- Facilities and ancillary providers: credentialing and contracting, including adding a new location.
- Dental professionals: credentialing and contracting, also requiring CAQH.
- Medical groups: AZ Blue says new groups with five or more practitioners may qualify for a group contract. At least one practitioner must be credentialed and the rest must be actively completing credentialing before the contract can be finalized.
- Chiropractors: credentialing is handled by American Specialty Health (ASH), not AZ Blue directly.
If a consultant or billing company is applying for you, AZ Blue requires you to complete and sign its attestation form before it will share information with or work with that company. Do this first so your application isn’t held up.
What you need before you open the form
The AZ Blue credentialing request site lists what to have on hand:
- Your CAQH Provider ID, with a complete and attested profile.
- Your NPI numbers: your individual NPI and, if you are in a group, the group’s organizational NPI.
- Your Arizona license number and the date you were first licensed in Arizona.
- Your DEA registration number, if applicable.
Two practical warnings from that page: you cannot save a partly completed form and come back later, so set aside enough time to finish it in one sitting. And physical, occupational and speech therapists who are part of a group need the group’s contract on file first before AZ Blue can complete individual credentialing.
Get your CAQH profile right
For professional providers, AZ Blue pulls your credentialing application from CAQH. Its operating guide says that if required information is missing from your CAQH profile, or your authorization for AZ Blue to view it has expired, the request is rejected and you have to resubmit. Within about a week of accessing your CAQH profile, AZ Blue screens it for items such as:
- A current certificate of insurance showing malpractice limits of at least $1M/$3M.
- Five years of work history, with an explanation for any gap longer than 12 months.
- A detailed explanation of any adverse history.
If something is missing, AZ Blue contacts the person listed on the request. According to the guide, if there’s no response within 30 days, the application is considered withdrawn. Our CAQH setup guide walks through building and attesting a profile.
Credentialing review and the contract
Once your file is complete, AZ Blue sends an acknowledgment and starts the credentialing review. A few dates matter here:
- Return the contract within 14 days. If you are eligible for an individual contract, AZ Blue sends it for review during credentialing. The guide notes that returning it late can delay your effective date. Receiving a contract does not guarantee participation, because it depends on passing credentialing.
- Answer follow-up questions within 7 days. If primary source verification turns up a question, the response is due within seven calendar days. Otherwise the file goes to the Credentials Committee “as is”, which can lead to an adverse decision.
- Committee decision. The Credentials Committee approves, denies, or proposes an alternative arrangement, such as a shorter recredentialing cycle.
Effective date: when you are actually in network
After approval, AZ Blue assigns an effective date. That date only applies if you have signed a provider participation agreement or are working under a group agreement. Services on or after the effective date are treated as in network. AZ Blue then countersigns your agreement through DocuSign and emails a welcome packet with the name of your Provider Relations Contact. Until you have that effective date, treat AZ Blue patients as out of network.
How long does it take? AZ Blue doesn’t promise a fixed timeline, and it depends on how complete your file is. Commercial insurers often take roughly 60 to 120 days from a complete application, and missing CAQH items are the most common reason for delays.
Staying in network
- Recredentialing at least every three years (every two years for urgent care facilities). AZ Blue warns that not responding to recredentialing requests will result in termination of the provider contract.
- Keep CAQH attested every 120 days, and keep AZ Blue’s access to your profile active.
- Report changes with the Provider Information Change Form.
What about AZ Blue’s Medicaid plan?
Blue Cross Blue Shield of Arizona Health Choice is an AHCCCS (Medicaid) health plan and has its own process. It uses the standard Arizona AzAHP Practitioner Data Form, submitted through the Health Choice provider portal, and it requires both completed credentialing and a signed contract. See the Health Choice provider page and our AHCCCS credentialing guide.
